This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part. Grade Determinations: Experience/Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). ["The VA Northern California Health Care system is currently recruiting a Social Worker Program Coordinator for the Community Nursing Home(CNH) program in the Geriatrics and Extended Care service at the Chico location. Major duties include, but are not limited to: The social worker will participate in the development, evaluation, and communication of standards, policies, and procedures, and plans of care for the CNH program. The social worker will educate medical center colleagues and/or students regarding the CNH program and assist with dissemination of information to other VA facilities and community partners. The social worker will evaluate CNH practices using relevant statutes and regulations and take action to improve identified deficiencies. The social worker is responsible for and will track CNH program assessments, consults, authorizations, oversight visits, data management, identify significant findings, problem solve, complete documentation reports pertaining to caseloads, productivity, and trainings. The incumbent must ensure that a comprehensive CNH data tracker is always up to date. The social worker will prepare reports and statistics for facility, VISN, and national use. The social worker will be responsible for conducting agency invoice audits as needed. The social worker will identify and develop program resources in the community, providing liaison with these providers. The social worker will provide program oversight to ensure quality care is provided by contracted agencies. The social worker will complete an initial screening of Veterans to determine whether they meet eligibility criteria for inclusion in the CNH program. The social worker will serve as a liaison between the contracted CNH provider and Veteran and/or family. The social worker is responsible for reviewing agencies which may be used to provide services to Veterans. Serves as the Contracting Officer Representative (COR) for all assigned CNH/C-ADHC contracts and fulfills all COR obligations. Obtains and maintains COR certification. Completes required performance evaluations for assigned contracts. Submits new and renewal acquisition packages to Network Contracting Office and collaborates with the assigned Contracting Officer(s) to ensure contracts meet the needs of the program and service. Reviews and approves claims for CNH/C-ADHC services in a timely manner. Collaborates with Community Care Service and Payment Operations Management to ensure that payments to CNH/C-ADHC facilities are made in a timely manner and in compliance with VA standards. Completes monthly face-to-face oversight visits to Veterans in Community Nursing Homes (CNH) and Community-Adult Day Health Care (C-ADHC) facilities. The purpose of oversight visits is for case management of Veterans and to provide direct/indirect services. Consults with CNH/C-ADHC facility staff to address and resolve Veterans' concerns as they develop. Ensures services are provided with attention to the dignity of the resident and consideration of the residents' capacity to understand treatment options. Provides review, follow-up, and appropriate documentation of all complaints regarding the program. Coordinates a quarterly satisfaction survey of CNH residents.
