The community you’ll be caring for:AdventHealth Hospice Care
The role you’ll contribute:
The APRN is responsible for evaluating and coordinating patient/family needs to enable preventative and supportive care under supervision of the Executive Medical Director.
The value you’ll bring to the team:
Provides medical care for hospice patients.
Utilizes assessment, intervention and teaching to maximize patient comfort and to enhance the quality of life to the patient/family unit.
Evaluates the health status of selected patients, orders treatment plans and provides education to patients and their families under the supervision of a licensed physician.
Performs clinical evaluations and reviews diagnostic studies of patients under the supervision of a licensed physician.
Participates in interdisciplinary team conferences of the facility teams, as needed.
Interacts with patients and families to manage the psychological, social and family issues regarding hospice and the services rendered.
The expertise and experiences you’ll need to succeed:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. A. Education and Training: Master’s Degree in Nursing with Nurse Practitioner designation is required.
B. Experience: A minimum of 3 years of experience in medical nursing, hospice and/or critical care experience and a minimum of 2 years experience as an APRN is required. Experience with symptom and pain management is preferred.
C. Certificates and Licenses Required: Clear/Active Florida APRN license required. Basic Life Support certification required. Certified Hospice and Palliative Nursing Certification is preferred
