St. Joseph Heritage Healthcare's PACE program is seeking a full-time nurse practitioner to join a compassionate and dedicated care team providing Palliative and Geriatric care services to patients 55 and older throughout Napa, California. Here, you will primarily focus on providing full-spectrum outpatient Palliative care services at a clinic, but you can also expect some time dedicated towards follow ups at nursing and patient homes as well.
