The Palliative Care Coordinator is a Master’s prepared registered nurse or social worker who possesses specialized training and expertise in the coordination of care for patients and families referred for Palliative Care services in the Colorado market. Delivery of palliative care services may be rendered in a variety of settings including but not limited to acute care, oncology, and/or specialty clinics. The Palliative Care Coordinator utilizes evidence-based practices to assist with the comprehensive healthcare assessment and management of the physical, emotional, and psychosocial issues related to acute, chronic and/or terminal health conditions.
The Palliative Care Coordinator provides leadership and coordination in the planning, organization, communication, and evaluation of all activities of the Palliative Care program. This person proactively identifies patients who are appropriate for palliative care/hospice services; assesses all palliative care and hospice referrals based on patient/family goals to determine appropriate level and type of care for outpatient palliative care/hospice services; provides information to patients/families regarding the palliative care/hospice benefit and insures continuity of care between settings, physicians and care team members; participates as a member of the interdisciplinary complex case management team; monitors and reports outcomes, assesses/identifies barriers to palliative care and hospice services and works with the interdisciplinary care teams to address such barriers; provides education to physicians, staff, and the community on topics related to palliative care, hospice services, and end of life care.

Coordinates the development of the palliative care program in the Colorado market as well as designs program modifications based on measurable outcomes and other continuous improvement activitie
Works as a member of the interdisciplinary complex case management team and consults with PCPs, pharmacist, and Medical Director on complex patient care issues
Proactively identifies appropriate palliative and hospice referrals and assesses all palliative and hospice referrals based on patient/family goals to determine which service and setting is most appropriate
Informs patients/families about the hospice Medicare benefit and ensures continuity of care between settings, physicians and care team members
Facilitates referrals to palliative care/hospice agencies
Provides education to physicians, clinical staff, patients/caregivers, and the community on palliative care, hospice services, and end of life care. This shall include, however not be limited to, palliative care and hospice protocols, patient/ family communication, cultural, psychosocial and spiritual concerns, Advanced Directives and community resources
Mentors, develops, inspires, and leads staff members in patient care and procedures settings, maintaining and promoting standards of care and development of critical thinking skills and role modeling advanced practice. Demonstrates the ability to perform behavioral competencies
Performs other miscellaneous duties as required
HIPAA Security Level: High access to systems/patient information
Works alternatively in office, patient care areas, and classrooms with little exposure to excessive noise dust and temperature
Occasional exposure to communicable diseases and exposure to body fluids
Office environment, some remote option. Full time Monday thru Friday 8 am to 5 pm. Some evening hours may be necessary related to program presentation
Must have the ability to push, lift, or pull objects up to 40 pounds, using proper body mechanics

5+ years current RN or Social Work experience in an acute care, palliative care, or hospice setting
2+ years of experience working in case mangement
2+ years experience as a member of an interdisciplinary team
Knowledge of palliative and hospice regulations
Knowledge of regulatory requirements necessary to integrate into patient care delivery system
Understand reimbursement for hospice and palliative care services
Ability to communicate effectively with patients and families regarding goals, treatments and services available including palliative and hospice care

Master’s degree in Nursing or Social Work-Current RN or Social Work license
National Certification in specialty area and/or Professional Certification in Palliative care or related certification

Colorado Residents Only: The salary range for Colorado residents is $54,400 to $97,000.
