North Wood Lodge Neighborhood Community Living Center is a 20-bed inpatient dementia unit, that is secured with an equipped wander guard unit, whose staff provides 24/7 skilled nursing care by RNs and Nursing Assistants. This is a skilled neighborhood specializing in the care of chronic psychiatric Veterans that must have a dementia diagnosis to be admitted to the unit. The resident population includes residents with a variety of medical and psychiatric diagnoses. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. The resident population includes residents with a variety of medical and psychiatric diagnoses that can include the following: acute and chronic depression, bipolar, schizophrenia, anxiety, substance abuse problems, diabetes, hypertension, congestive heart failure, and wound care treatments. The incumbent assesses, plans, implements and evaluates care based on age-specific components. She/he assumes responsibility for the coordination of care focused on resident transition throughout the continuum of care, resident and family education, resident self-management after discharge, and supporting factors that impact customer satisfaction. Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. The RN will be held accountable to the full scope of their licensure. The RN possesses the knowledge and skills necessary to effectively apply all aspects of the nursing process within a collaborative, interdisciplinary practice setting. This broad based clinical knowledge enables the RN to effectively implement health promotion and prevention interventions, manage acute and chronic illness states, assist Veterans in the attainment of optimal levels of functioning through rehabilitation, and provide supportive measures for the dying Veteran and their significant others. On a daily basis all Registered Nurses in this position are expected to perform the usual and customary duties of a front-line nurse including but not limited to physical assessment, wound and skin assessment/ care, medication administration, and general nursing management of the residents physical and emotional well-being. The nursing process of assessment, planning, implementation and evaluation is applied on an ongoing, systemic manner to all residents. Registered Nurses are actively involved in providing, developing and facilitation resident education. The unit and nursing staff also support other areas within the Medical Center as needed and accompany residents upon transfer to other facilities if necessary.
