Job Summary:  Under the direction of the Director and in collaboration with the Supportive Care Nurse Practitioner, the Nurse Navigator is responsible for overseeing, coordinating, and educating the FHN Supportive Care patient population. Triaging phone calls from patients and families, staff, providers, and other healthcare workers internally and externally and addressing their needs and concerns promptly to assure high quality care. Obtains information on patient referrals, reviews the medical record and communicates with the Supportive Care Nurse Practitioner to make an information visit and/or admit the patient or performs the info/admit directly.
