Providence ElderPlace is seeking a part-time nurse practitioner or physician assistant to join its team in Portland, Oregon. You will be part of a program that provides older adults with comprehensive care and social services. You can expect to provide care in a variety of settings including at medical clinics, private residences, and community-based facilities. Be part of an compassionate team of care givers providing high-quality services to older adults throughout the Pacific Northwest.

Monday to Friday schedule, daytime hours (.8 FTE)
Outpatient primary care setting with home and community facility visits
Highly collaborative team
At least 1 year of geriatric experience is required
2 years of geriatric post-graduate clinical experience preferred

Providence ElderPlace is a Program of All-inclusive Care for the Elderly (PACE), and is the first program of its kind in Oregon. With eight conveniently located centers in the Portland area and on the Oregon Coast, Providence ElderPlace provides and coordinates all necessary care services for seniors to remain active within their communities. The PACE model offers medical care, physical and occupational therapy, lab tests and procedures, home care services and more.
