The Home Telehealth Registered Nurse (HT RN) uses a wide variety of communication and information technologies to ensure excellence in the health care delivered to our nation's Veterans along with the nursing process and evidence-based practice to collaborate with the Primary Care Teamlet (Patient, Primary Care Provider, RN Care Manager, Clinical Associate and Clerical Associate) and expanded Team (family/caregiver, internal and community based services).
