The Home-Based Primary Care LPN/LVN functions as a team member under the clinical guidance of a Registered Nurse. The incumbent is self-directed and functions in a highly independent role within the home care setting. The HBPC LPN/LVN is responsible for prioritizing patient needs and adjusting plans for delivery of care. The incumbent demonstrates the ability to observe, identify and respond to the patient's nursing care needs, recognizing deviations from baseline. Basic Requirements: Citizenship.
