The Home Based Primary Care (HBPC) Medical Director, is responsible for the overall medical care delivered by the HBPC team. to ensure the care for many Veterans with multiple chronic illness who are at high risk for poor outcomes, that are enrolled in the HBPC Program, receive comprehensive, interdisciplinary, primary care in their homes, for Veterans with serious medical, social, and behavioral conditions for whom routine clinic-based care is not effective or may not be possible.
