Tampa VA Medical Center is looking for Social Worker (Program Coordinator). The Community Nursing Home (CNH)/State Veteran Home (SVH) Coordinator is a Licensed Clinical Social Worker administratively responsible for managing the daily operations of the CNH program as well as direct patient care to those who reside in the contracted nursing home facilities. Applicants pending the completion of educational, or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates.GS-12 Social Worker (Program Coordinator) candidates must have the following: Experience/Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure. Advanced practice social worker who can be further differentiated from the independent practice social worker by the incumbent's ability to expand clinical knowledge in the profession, provide consultation and guidance to colleagues, role model effective social work practice skills, teach or provide orientation to less experienced social workers, develop innovations in practice interventions, and provide clinical supervision for social work license or certification. The Social Work Coordinator is required to be in the community to provide oversight and care coordination to Veteran's receiving care at local hospitals, rehab facilities and at other community partner locations. The use of a government vehicle is required. The incumbent must have a valid state drivers license and pass a medical exam to drive government vehicles. Duties Include but not limited to: Responsible for the overall function of the CNH/SVH programs. Is responsible for the recruitment, growth, quality management and quality oversight of new and existing facility partners/care locations. Functions include the following: the national program field guidebooks and handbooks, development and management of local policies or practice/participation guidelines, management and coordination of nursing homes, quarterly contractor office representative duties (COR), case management, development of policies and care processes, inspections and coordination with various interdisciplinary treatment teams throughout the VA Medical Center. Establishes and maintains a relationship with the facility team, Veteran, and family members. Receives and completes consults from interdisciplinary teams and other social workers on complex, difficult cases, using advanced practice skills and expertise. Provides consultation to referral sources and is responsible for development and maintenance of professional standards of practice and educating others regarding such. Interprets and explains VA treatment and benefit programs. Serves as a resource of knowledge of other programs and services from agencies that may enhance the quality of life for Veterans and makes appropriate referrals to said agencies. Documents independently in the medical records utilizing paper electronic format, including psychosocial assessments, diagnostics, treatment, progress notes, follow up and referrals. Has extensive knowledge of medical center policies and procedures, VHA and VBA regulations including the national handbooks/field guidebooks for the programs (CNH/SVH). Ensures the direct supervisor, interdisciplinary team, and facility leadership are kept informed. May provide outreach and education on the programs and other VA services, which are designed to increase Veteran and general community knowledge of VA services. Serves as a mentor to other social workers, peers, employees, health care providers and partners. Provides training to caregivers of the facilities, students, residents, and interns from other disciplines regarding the benefits of a VA services and local community resources. Can communicate effectively, both orally and in writing, and with people from varied backgrounds. Must provide oversight and clinical supervision to Social Work Trainees and/or GS-9 Social Workers or Licensure (if State license allows and per SW qualification standards). Will represent the VA in the community in a positive and professional manner. Work Schedule: Monday through Friday, 8:00 am to 4:30 pm, with rotating tours of duty as needed.
