This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Home care experience ICU experience Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. ["The incumbent practices as an RN in Home Based Primary Care (HBPC) at the Alaska VA providing nursing care in the home to a culturally diverse Veteran population with complex, chronic, and disabling conditions. Performs comprehensive case management providing initial and ongoing assessment of unique nursing needs of the patient and caregiver documenting and reporting any significant changes in the Veteran's medical condition to the provider. Evaluates patient's response to treatment, anticipates patient care needs and initiates appropriate changes to plan of care. Demonstrates ability to address and analyze complex, difficult and intractable problems and implement effective and innovative solutions. Documents patient findings, assessment and care provided in a timely manner. Assesses patient's learning needs, develops and implements an individualized patient centered plan to address patient care goals, and instructs patients and families. Evaluates and modifies approach based on knowledge of human behavior, motivations and patient responses to situation. Provides age-specific care for adults {18-75) and geriatric (over 75) patients including the following elements of care related to the aging process: promoting independence; encouraging self-care; involving family and/or designated caregiver with care; providing necessary safety measures; and assisting patient to cope with and/or compensate for changes affecting mobility, balance, losses, cognition, eyesight, hearing, skin, bones and thermoregulation. Assesses age-specific care requirements and develops an appropriate plan of care which incorporates age specific needs. Conducts self in manner that demonstrates adherence to standards of professional practice and patient care. Engages HBPC team in identifying, analyzing and resolving patient care problems. Demonstrates knowledge of types and scope of patient resources available through the VA and community based programs. Plans patient care for maximum productivity, efficiency and positive outcomes. Assesses ongoing workload management, delegating and adjusting work responsibilities to meet needs of the patient and HBPC Program. Identifies current and potential safety issues and takes actions to prevent adverse events.
