Position Summary:  Physicians in the Home Visit Physicians practice provide primary medical care to patients in their homes and facilities. Essential Functions: Provide primary medical care to Home Visit Physician (HVP) patients. Collaborate with other clinical staff in the HVP practice to insure quality patient care. Provide appropriate communication and coordination with any of the patient's specialists on an as needed basis. Maintain open communication with office staff related to scheduling and other patient care needs. Complete documentation in EMR in timely manner. Support and participate in performance improvement and quality assurance initiatives in the practice.
