Here, you will work with a well-established team providing older adults comprehensive care and social services while also managing a small panel of patients at a primary practice location. Be part of a passionate team of care givers caring for older adults throughout the Pacific Northwest. Outpatient primary care setting, home visits and visits in community-based settings Collaborate with physicians, nurse care manages, social workers, pharmacists, behaviorists, and other team members 2-years of post-grad clinical experience working with geriatric population or hospice population preferred
