Additional Information About the RoleJoin BJC's new exciting and well supported initiative - "Evelyn's House at Home" where hospice patients receive an increased level of support to settle in at home, receive patient and family education and ensure a comfortable home environment.  This is your chance to make a difference in end of life care by providing supplemental home support with Evelyn's House level of care.  The progam allows hospice patients with intensive symptoms to the ability to remain at home for hospice.  Patients receiving "Evelyn's House at Home" care supports patients who have uncontrolled symptoms they are trying to manage at home with continous care.
