Providence Hospice of Petaluma is seeking a full-time nurse practitioner to join a compassionate team. This position will be supporting a team of RN's as well as a Hospice physician by performing symptom management, med management, and visits for recertification to end of life patients. The role requires candidates to provide at-home services as well as travel to various long-term care facilities throughout the area.
