JOB SUMMARYThe Care Coordinator Licensed Vocational Nurse (LVN), under the direction of the Registered Nurse (RN), is accountable for assessing and coordinating patient care across the continuum utilizing accepted criteria and guidelines to coordinate benefits and resolve medical necessity of services provided.
