The Social Worker performs specialized treatment in the area of Home-Based Primary Care (HBPC). Functions include working with the interdisciplinary Home-Based Primary Care Team to coordinate care needs and to provide appropriate social work intervention for all consults to the Home-Based Primary Care, and to provide counseling, case management and education for Veterans and families followed by the Home-Based Primary Care Team. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
