The Registered Nurse (RN) Home Care Coordination (HCC) position is located in the Geriatrics & Extended Care (GEC) Service at the West Roxbury Campus location. This position is full-time at 40 hours per week. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Physical Requirements: Position is subject to a pre-employment physical and medical examination. This position requires: lifting, carrying; reaching above shoulder; use of fingers; walking (8 hours); standing (8 hours); repeated bending; working alone; protracted or irregular hours of work and occasional emotional strain. It also requires intermittent lifting, carrying, straight pulling and straight pushing up to 45 lbs. The RN provides initial and continued assessment of nursing needs of the patients, evaluate cardiopulmonary status of patients, teaches the patients and caregivers effective and efficient ways of managing and delivering care in the home, monitors the patients' condition and the care provided in the home, provides direct nursing care - to include but not limited to wound care, medication management, and extensive patient teaching. The incumbent provides care management and care coordination for the HCC patient, monitors the patients' nutritional, psychological, and physical status. The RN demonstrates ability to clearly narrate patient assessments to the physician: assess heart and lung sounds, hemodynamic status, assess for edema, GI/GU assessment, wound assessments with the intent of assisting the Veteran to transition to ongoing longitudinal care in PACT or HBPC (as appropriate).. Duties include, but are not limited: Demonstrated clinical competencies in such areas as age-specific care, cultural assessment, and health education. An ability to effectively function autonomously, as well as a member of an interdisciplinary team. Respect for cultural and spiritual values. A commitment to discipline-specific standards of practice, including ethical conduct, the primary care delivery model, chronic disease management, palliative care, principles of care management, a holistic framework of practice, and safe provision of care via infection control, equipment, and environmental safety. Determine the appropriateness of patients for admission into the HCC Program. Help develop patient treatment plans based on a comprehensive interdisciplinary assessment of the patient and the caregiver. Implement the patient's treatment plan through home visits. Review the patient's progress as a team at regular intervals, at least twice weekly, or when there is a change in the patient's condition. Assess and support the caregiver's capability to continue to provide care. Educate the patient and the caregiver. Coordinate the process of care with the other team members to maximize efficiency. Assess the patient's continuing need for HCC care, and plan for HCC discharge when indicated. Participate in HCC team meetings. Case management of assigned patients, to be determined by nurse manager. Performs cross coverage of other HCC divisions as needed, and other duties as assigned by nurse manager.
