The Hematology/Oncology Advanced Practice Registered Nurse (H/O APRN) is responsible for providing care to patients in the Hematology/Oncology Clinic, utilizing nationally approved guidelines and standards of care. They will act as an advocate and direct patients with suspected hematology, oncology conditions through the diagnostics phase of medical care. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Citizen of the United States. (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.) English Language Proficiency. Direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d). Nurse Practitioners. A nurse practitioner must be licensed or otherwise recognized as a nurse practitioner in a State, possess a master's degree from a program accredited by the NLNAC or CCNE, and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. [The certification must be in the specialty to which the individual is being appointed or selected.] Current, full, active, and unrestricted license registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Ensures that the clinical testing and follow-up occurs in a timely manner. Serves as a liaison between the patient and multiple providers, including the primary care service, radiology, specialty services, and cancer-related specialties. Develops of treatment plan based on the available clinical findings and algorithms and communicates the plan of care to the patient and appropriate care providers. Continuously reviews and revises patient care based on new clinical findings and assessments while following approved clinical protocols and privileges. In addition, assists with the development of new protocols or clinical pathways to standardize care for patients with hematology/oncology conditions to expedite patient processing and care delivery. Focuses patient education on the causes of hematology/oncology conditions and provides therapeutic counseling/consults when clinically indicated. Develops a tracking mechanism for all patients at our facility undergoing surveillance screening. Monitors and assists with follow-up care of abnormal findings including referral of any incidental findings. Reviews all laboratory, radiology reports, diagnostic testing, consultative reports for patients seen in clinics and reports these results to patients as required by VA directives. Screens patients for disease prevention and identifies health risks, for hematology/oncology conditions. Orders laboratory, radiologic and other diagnostic tests and provide initial interpretations. Assesses patient's learning needs, development, and implementation of plans to meet needs, and instructing patients and families in self-care and treatment. Participates in the operations of the Hematology/Oncology program. Supports activities to identify appropriate patients for clinical screening. Monitors patients due for screening and assist with scheduling screening scans and follow up procedures. Anticipates, identifies, and helps patients to overcome barriers within the VA healthcare systems. Assists in coordination of healthcare services and referrals. Maintains awareness of patient's plan of care and communicates as necessary issues that arise to prevent unnecessary delays in treatment, including missed appointments. Organizes and leads in-services to share information about the Hematology/Oncology program with clinical staff. Serves as an operational resource for clinical staff involved in the program. Works with the local clinical applications coordinator to ensure that any clinical reminders and templates are reviewed, updated, and implemented appropriately.
