The Lung Cancer Screening Clinical Coordinator position is located within the Pulmonary Section of the Department of Hospital and Specialty Medicine (DHSM) at the VA Portland Health Care System (VAPORHCS). The Clinical Nurse Coordinator will organize and coordinate the implementation and day-to-day operation of the VISN 20 Centralized Lung Cancer Screening Program. Assures adherence to protocols, procedures, and program plans established for the VISN 20 Centralized Lung Cancer Screening Program. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: BSN or higher; at least 2 years of nursing experience; pulmonary, care coordinator/case management and healthcare informatics tools experience. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. ["The program was originally established as part of a national, multi-site clinical demonstration project conducted by the VHA National Center for Health Promotion and Disease Prevention (NCP). The Clinical Coordinator will work under the direction of the VAPORHCS and VISN 20 Centralized Lung Cancer Screening Program leaders to track and coordinate ongoing patient care, while collecting, analyzing, and reporting program evaluation data for assessment on the impact of implementing lung cancer screening and to make evidence-based improvements. The incumbent serves as an operational resource for VISN 20 lung cancer screening team members, which may include clinical staff from primary care, mental health, pulmonary medicine, cardiothoracic surgery, pathology, medical oncology, imaging, and radiation oncology. Functions include but are not limited to: Manages consults for potential lung cancer screening enrollment by performing chart reviews to verify eligibility and appropriateness. Conducts shared decision-making interactions with potential lung cancer screening enrollees. Organizes and leads in-services to educate and share information about the Lung Cancer Screening Program with clinical staff. Collaborates with informatics team members to ensure that program clinical reminders, templates, and management tools are implemented appropriately. Facilitates follow-up referrals to evidence-based smoking cessation care. Optimizes patient-centered care practices through patient education, care management, screening exams, procedures, and clinical appointments. Maintains accountability for following imaging studies and their recommendations. Collects and analyzes program data from a variety of sources. Provides regular communication and reports as needed to the Lung Cancer Screening Program team. Identifies protocol problems and assisting in problem resolution efforts. Performs initial and ongoing monitoring and tracking of abnormal images/lung nodules. Communicates with clinicians to ensure that they are alerted to abnormal findings when necessary. Covers other assignments and/or managing other caseloads as requested for the Unsuspected Radiological Findings and Lung Cancer Screening programs within VISN 20. The incumbent requires the following competencies: 1) excellent interpersonal skills and abilities to work effectively in a team setting; 2) independence, initiative, and flexibility to function in an evolving role that crosses the continuum of care and service lines; 3) excellent verbal, written, and electronic communication skills; 4) computer skills in the current electronic health record, informatics tools, word processing, and spreadsheets; and 5) knowledge and skills to collect, analyze, and/or interpret patient care data for trends, variance, and costs.
