VA Portland Health Care System is a Complexity Level 1a Facility and provides extensive primary, secondary, tertiary, and quaternary care in outpatient, inpatient, residential, and community settings. VAPORHCS is a multi-site medical system that includes a major medical facility in Portland, Oregon, outpatient and residential facilities in Vancouver, Washington, and 10 Community Based Outpatient Clinics. Education (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations:GS-14 (a) Experience. In addition to the GS-13 requirements, must have 1 year of experience equivalent to the next lower grade level. Assignments. Candidates at this grade level are to be in one of the assignments listed below. . For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time. He/She serves as the Acting Service Chief in the absence of the Service Chief. The incumbent develops and maintains a system of internal reviews that ensure service programs operate at a satisfactory level of performance and in compliance with regulatory and accrediting bodies. He/She is responsible for supervising, management, and coordination of comprehensive pharmaceutical care activities in the inpatient setting which included the acute hospital, Ambulatory Surgical Unit, procedures areas, as well as the Community Living Center (CLC). These activities include the acute care clinical pharmacy program, inpatient 24/7 pharmacy distribution services (unit dose, IV admixtures, ward stock, controlled substance, prescriptions, etc.), and inpatient pharmacy inventory and supply management. The inpatient pharmacy serves the inpatients and staff at VA Portland 24 hours a day, seven days a week; provides prescription services to patients upon discharge from hospital and to patients from the Emergency Department (ED) when the outpatient pharmacy is closed. Inpatient Pharmacy is responsible for more diverse and complex services than any other section. The manager is responsible for the supervision and management of those services and the personnel that provide them. In addition, the manager is also responsible for ensuring that inpatient pharmacy services and operations satisfy DEA, FDA, VHA and VISN regulations and policies, as well as CARF and TJC long term care and acute care requirements. The duties of this position include but are not limited to the following: Shares the responsibility for ensuring compliance with a wide variety of regulatory requirements. VHA policy must establish a foundation for local medical center memorandums (MCMs). The MCMs will ensure compliance with VHA Directives and Handbooks, Drug Enforcement Agency (DEA), The Joint Commission (TJC) and the Office of the Inspector General (OIG). Compliance with the MCMs will include: TJC standards, medication management, environment of care, and applicable National Patient Safety & National Pharmacy Benefits Management goals that are integral part of pharmacy planning, policies, and procedures. Ensures that the Medical Center and Pharmacy Division policies and procedures related to Clinical Pharmacy are reviewed and/or revised in a timely manner before expiration or as changes occur. Creates new policies when required. Demonstrates knowledge of medical center memorandums, policies and procedures related to pharmaceutical services and thorough knowledge of Pharamcy Service policies, procedures, memorandums, and directives applicable to areas of responsibility. Plans, directs, manages, monitors, evaluates, enhances, and promotes Inpatient Pharmacy Services provided by the Pharmacy Division. Oversees the effective utilization of resources and budget to support the mission and goals of the organization. Utilizes staffing standards, creates staffing models and assesses workload capture information meet productivity and provide safe, efficient care. Promotes a patient centered, team- based multidisciplinary model of pharmaceutical care to promote drug prescribing, medication monitoring, and drug laboratory review that assures positive outcomes from medication use. Provided development, enhancement, and oversight management for VISN 20 Virtual 24/7 Pharmacy operations at sites of care within the Oregon catchment area including but not limited to Roseburg and White City VAs. Assures inpatient medication order verification for VISN 20 for patients treated after normal hours of pharmacy operation at these sites. Provides expertise in drug interactions and incompatibilities via consultation with physicians, nurses and patients.
