Schedule:FT, 4 x 10 hour Days
Shift:
Hours for training will be M-F
once trained it will shift to Friday-Monday, 10 hour days.
Training will be in person, working remotely after training is possible.

The role you’ll contribute:
Responsible for preparing and maintaining schedules for clinicians under the direction of the Patient Care Supervisor. Perform other secretarial/clerical/clinical duties as directed.
Coordinates receipt of referrals within the agency by conducting an initial phone evaluation for appropriateness of and acceptance of the referral for home health services. The coordinator will collect patient clinical and demographic data, insurance or payer information, verify payer coverage, obtain physician orders and arrange for support services. Written orders for homecare will be obtained when possible. An RN will be requested to take verbal orders for homecare services when needed.

Prepare and maintain schedules for all clinicians, in collaboration with Administrator on-call for the weekend.
Convey visit management information between field clinicians and PCS, assuring current communications with patients and facilities as well.
Be fully cross-trained in clerical office activities, to provide back-up/coverage for all roles as appropriate (to include but not limited to Intake, Payroll, Authorization, etc.)
Coordinates the intake of all referrals into the agency. Serves as the central coordination point for all referrals coming from hospitals, physician offices and any other referral source.
Verifies that physicians ordering homecare are licensed and able to participate in the Medicare program.
Performs care coordination activities upon receipt of patient referral; assesses if homecare services are appropriate, verify/obtain physician orders, ensure payer coverage is verified, coordinates arrangements for HME, infusion and other needs.
Assures referrals have been entered or enters referrals into the computer, verifies that required information is present prior to admission; maintains accurate reference files such as physicians, payers, and referral source information.
Performs and documents payer verification process; verifies Medicare eligibility coverage and insurance or self pay verification for patients prior to admission; follows policy for charity approval process.
Functions as liaison between clinicians in facilities and clinical teams to ensure patients are scheduled in a timely manner and the location/status of all patient referrals is tracked.
Maintains logs as needed for patients who are hospitalized or in skilled facility, new and resumption of care referrals, etc.)
Monitor and supervise the daily clerical operations on the weekend of the home health office and assures duties are performed within established guidelines in accurate and timely manner.
Assigns/delegates responsibility to appropriate staff
Qualifications
The expertise and experiences you’ll need to succeed:
KNOWLEDGE AND SKILLS REQUIRED:
Knowledge of computer and standard software applications, appropriate to position and agency required.
Capable of multi-tasking, in a fast-paced working environment required.
Able to communicate effectively in English, both verbally and in writing.
EDUCATION AND EXPERIENCE REQUIRED:
Associate or Bachelor’s degree in business or medical related field
Minimum of 2 years of experience in health care setting/medical office required
Experience working in a Medicare certified homecare agency
