Function as a Geriatric Clinical Specialist in the multidisciplinary team to provide recovery oriented, solution focused, client centered case management services and other general therapeutic procedures to clients in skilled nursing facilities in the community. Using a client focused, recovery oriented, strength based approach to care in which we work jointly with our clients, their families, and their support systems to develop and deliver holistic services that are both needed and wanted. In doing so, we balance strength, desire, hope, independence, and choices with safety, support, and acceptance. Minimum Requirements: 2 Years of Experience preferred
Must be able to provide ongoing coordination of case management services, assessment, treatment planning, resource identification and development, environmental assistance, advocacy, service monitoring and follow-up, collateral supports and clinical services as appropriate to assigned caseload.
Must be able to provide services utilizing this approach with respect, tact and sensitivity. Must be able to tolerate the stress of managing difficult/agitated clients in a safe and dignified manner as dictated by facility policies and procedures.
Must be able to work cooperatively in a multidisciplinary team approach, according respect to others based on knowledge and experience as well as discipline.
Provides clinical services and treatment as per Washington Administrative Code.

Master's Degree in Behavioral Sciences, at least two years related experience must be in Social Services working with the mentally ill.
Geriatric Mental Health Specialist preferred.
Current Registered Counselor in the State of Washington.
Current Washington State Driver's License with proof of automobile insurance.
