Position Summary:
The Home Health Admissions Nurse is primarily responsible for completing the initial assessment of patient and family to determine home health needs. This position performs comprehensive assessments to include physical, medication reconciliation and OASIS; initiates plan of care and establishes initial visit frequencies; based on policies and procedures, through the competent application of the nursing process in accordance with the Registered Nurse Practice Act, State and Federal Regulations. The Admissions Nurse uses critical thinking skills in making decisions regarding client care, and assigns the performance of selected client care to competent individuals in selected situations. Assigns tasks and retains the responsibility and accountability for the care and coordination of care for the client. Acting as the client advocate, the Admissions Nurse has continuous regular contacts with physician, clients, family, caregivers, community agencies, and others involved in the client’s care.

Minimum Requirements:
One year medical or surgical nursing required
One year Home Health experience preferred

Education/Licenses/Certifications:
Bachelor's degree in Nursing, or obtained within three years from the initial date of hire
Licensed in the State of Washington as a Registered Nurse
Possession of current CPR Certification
Current Washington State Driver’s License and Automobile Insurance
