The position will require travel to the Fonda catchment area. You will be responsible for providing comprehensive, interdisciplinary, primary care in the homes of Veterans with serious medical, social, and behavioral conditions for whom routine clinic-base care is not effective or may not be possible. The HBPC Veteran population consist of those facing the challenges of disability, aging, and chronic disease that are at high risk of hospital, nursing home, or recurrent emergency care. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. This Home Based Primary Care Nurse Practitioner position is located in the Primary Care Service at the Albany Stratton VA Medical Center, Albany, NY. The Nurse Practitioner applies the nursing process and care management principles within a collaborative, interdisciplinary practice setting. Nurse Practitioner is supported in the provision of complex patient care by utilizing techniques to effectively implement health promotion and prevention practices and the management of acute and chronic illness of the HBPC patient.
