Department Overview

The Home Care and Hospice department at Children's provides care and support for patients who live in the Twin Cities seven-county metropolitan area. The team is made up of registered nurses, clinical social workers, child life specialists, chaplains, and intake coordinator who work collaboratively to provide care in the patients home. Services provided include home care and home care pharmacy, hospice care, palliative care, perinatal hospice program, and Children's Institute for Palliative Care.


Position Summary

Children's MN Intake Resource Nurse is responsible for all clinical aspects of the Home Health and Hospice referral intake process; ensuring compliance with all State, Federal, and Joint Commission regulatory requirements. This role establishes and maintains positive relationships with internal customers and referral sources by interacting and developing relationships that promote the continuum of care.

Responsibilities include screening newly referred patients for the appropriateness of skilled nursing visits, home infusion services, palliative or hospice care, ensuring timely communication and coordination in order to meet with needs of patients and their families. Performs duties and responsibilities in a manner consistent with our mission, vision, and Children's MN values.

Qualifications

:
* Current Minnesota RN License required.
* BSN preferred.
* Current BLS Certification required.
* Pediatric experience required.
* Experience applying clinical considerations to identify home service needs in relation to clinical cares, treatments, procedures and patient/family education.
* Three years of community health nursing experience in a certified home health agency is preferred with two years of recent clinical experience.
* Excellent interviewing, assessment, communication and negotiation skills.
* Proficient experience using electronic medical records.
