OPPORTUNITY HIGHLIGHTS
St. Peter's Health Partners is seeking Board Certified Nurse Practitioners with interest and experience in Geriatrics, Hospice, Palliative Care, and/or home healthcare to join the Home Based Primary Care team located in the Capital District, New York.
Our Home Based Primary Care Team manages the care of homebound patients residing in Albany, Rensselaer, and southern Saratoga counties. Our team includes three Nurse Practitioners working with a collaborating physician experienced in the care of chronically ill/geriatric patients. One NP serves as the Practice Manager in addition to managing a caseload of patients, and the other 2 NPs have a caseload of assigned primary care patients within a designated geographic area/county. The NPs are supported by a full time Senior Registration Associate and a full time RN Care Manager. In addition, per diem NPs assist with on-call rotation, one covering most weekends and one assisting in the rotation. This program exemplifies the Triple Aim in healthcare: better health/outcomes, better patient/family satisfaction, and lower total cost of care.
Our providers serve as the patient's primary care provider, either long-term or on a transitional basis (3-4 months) to stabilize readmissions and then return to the patient's original office-based primary care provider. Each provider manages a caseload of approximately 100-120 homebound patients, 80 years of age upon admission on average, 77% with advanced illness/multiple chronic conditions, 17% bariatric, 12% without a current primary care provider and many not having been seen by their primary care provider in a while due to difficulties getting to the office.
Our team works closely with providers across SPHP Medical Associates in the Medicare Shared Savings program and collaborating in the care of those who are homebound to ensure access to primary care. The team's office is co-located with our comprehensive home health services to facilitate close communication with an interdisciplinary team involved in the patient's care.
Requirements for this opportunity include:
Board Certified - Adult Gerontology or Family Nurse Practitioner Certification
NYS License and DEA certification
At least 2-3 years of Nurse Practitioner experience working with an elderly population is required
Experience working as a Nurse Practitioner in a Primary Care setting is preferred
