Position Summary: To assess the nutritional status and dietary needs of PNW PACE program participants and plan for the provision of adequate dietary intake to enable client to achieve his/her optimal level of independent functioning. To provide necessary consultation and teaching to participants, families and the health team. To oversee food services, including congregate and home delivered meals. All PNW PACE staff members must demonstrate the knowledge and skills necessary to assess, plan care for and provide service to frail elderly participants according to the assigned responsibilities and PNW PACE standards. Internal contacts include physicians, interdisciplinary team and other PNW PACE staff. External contacts include participants, their families or responsible parties, physicians, community health partners, and vendors.
Minimum Requirements: Prior experience with geriatric population preferred Prior experience with home health preferred
Education/Licenses/Certifications: Bachelors Degree in food & nutrition, dietetics, or related field Registered Dietitian with the American Dietetic Association or obtained within 6 months of hire.
