This Nurse Care Coordinator position is a full time position, M - F. The incumbent will be working with the Nurse Practitioner, a social worker and a team of physicians who all specialize in Palliative Care. Incumbent will also collaborate with all Specialties across the organization to provide the best care.The ideal candidate will have a minimum of 2 - 5 years of experience in the in/outpatient care; working with patients who are medically complex; Palliative Care experience is a plus.
