Location: Home Health visits in Jackson or Clay County depending on RN.
The role youâ™ll contribute:
The Home Health RN Nurse is a professional nurse who under the general supervision of the Care Manager. Provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization. This position is accountable for providing professional nursing activities as identified and planned by the R.N. Case Manager in the Patient Care Plan to maximize function within the home restoring health or assisting a peaceful death. May be on call for after-hours work. Coordinates and directs the home care patientâ™s care based on individual patient needs.

Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Organize and implement individual patient care based on ongoing interpretation of physiological, psychological, sociological deficits affecting the state of health.
Qualifications
The expertise and experiences youâ™ll need to succeed:
Minimum qualifications:
Graduate of an approved school of professional nursing
Have at least one (1) year of institutional experience
Maintain professional licensure of a Registered Nurse in the state.
Current CPR certification
Excellent oral and written communication skills
