Career Center

• Coordinate with patient/family plan of care with an interdisciplinary team that includes Physicians, Advanced Practice Providers, social workers and pharmacists to promote smooth transitions in case and respect for the patients care choices.
• Provide consultative visits to patients in the hospital requiring palliative care interventions; including goals of care discussions and symptom management assessment and interventions.
• Will collect health status data on palliative care patients relevant to his/her presenting problem in a continuous, systematic and complete manner.
• Will collaborate with hospice and palliative care colleagues to ensure smooth day-to-day operations within the teams. And, that the care provided to patients meets agency and regulatory quality standards, with effective and efficient resource utilization.
• Monday- Friday Schedule, no weekends, no call coverage
• Average daily patient census 4-6 encounters per day for inpatient consultations and new patient follow ups.
• Exclusively the only Joint Commission Certified Palliative Care team in the University of Maryland Medical System and #2 Palliative Care team in the state of Maryland!
