4-Hour Shift When Scheduled
Summary:
The Home Health Care Aide provides personal care, assists with food preparation, assists with exercises, and provides other activities of daily living as ordered by the physician.
Responsibilities:
1.Assists, prompts, and/or provides direct personal care and assists with food preparation and other activities of daily living (ADLs) in order to maintain proper hygiene and nutrition for select patients.
2.Performs simple procedures as an extension of therapy or nursing services as indicated in the patient plan of care may assist in administering medications that are ordinarily self-administered.
3.Assists with educational activities and habilitation, if indicated on the patient plan of care, to the extent allowable by the patient's payer source.
4.Assists select patients with homemaking/chores if indicated in plan of care and allowable by payer source.
5.Documents all care given to patient in the patient medical record.
6.Demonstrates the knowledge and skills necessary to provide care for the physical, psychological, social, educational, and safety needs of the patients served regardless of age.
7.Other duties as required.
Other information:
Technical Expertise
1.Good communication skills are required, including the ability to read, write, and verbally report clinical information.
2.Experience in Pediatrics and/or working with individuals with developmental delays is preferred.
3.Experience working with all levels within an organization is required.
4.Experience in healthcare is preferred.
5.Proficiency in MS Office [Outlook, Excel, Word] or similar software is required.
Education and Experience
1.Education: High School Diploma or equivalent is required.
2.Certification: CPR and First Aid certification is required. Home Health Aide certification is preferred or upon hire must successfully complete a Home health Aide training program which includes a minimum of 75 hours classroom and supervised practical training
3.Years of relevant experience: 0 to 2 years is preferred.
4.Years of experience supervising: None.
