The Wound Care Nurse Practitioner/Clinical Nurse Specialist acts as a consultant to outpatient, inpatient, resident, primary care, home-based primary care, medical and nursing staff in assessing, planning, implementing, evaluating and reassessing the care of patients with a variety of wounds and stomas, to include but not limited to lower extremity wounds, abdominal stomas, draining wounds, fistula, pressure ulcers, dermal wounds, acute surgical wounds, and incontinence issues. Advanced Practice Nurse (Nurse Practitioner/ Clinical Nurse Specialist) Basic Requirements English Language Proficiency. In accordance with 38 U.S.C. 7402(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Education: A post-master's certificate or master's or doctoral degree as a NP or CNS. (Applicants pending the completion of educational requirements may be referred and tentatively selected but may not be hired until the education requirements are met.) Must be a graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). NOTE: Foreign Education. Graduates of foreign schools of professional nursing programs, the nurse practitioner degree equivalency must be validated through an Agency recognized degree equivalency evaluation agency, such as the International Consultants of Delaware (ICD). Candidates with equivalent nurse practitioner masters or doctorate level degrees who possess current, full, active, and unrestricted registration and certification will meet the requirement of graduation from an approved school of professional nursing to warrant appointment. License and Certification: License. All APN (NP/CNS) must possess and maintain an active, current, full, and unrestricted APN license to practice as an NP/CNS in a State, Territory or Commonwealth of the United States, or in the District of Columbia. Basic Certification. All APN (NPs) must maintain full and current board certification in an area of clinical specialty from one of the following: the American Nurses Credentialing Center (ANCC), American Academy of Nurse Practitioners Certification Board (AANPCB), or other nationally recognized certifying body in the area in which the Nurse Practitioner is academically prepared or Basic Certification. All APN (CNSs) must maintain full and current board certification in an area of clinical specialty. Certification must be obtained through the American Nurses Credentialing Center (ANCC), American Association of Critical-Care Nurses (AACN), or another nationally recognized certifying body. The following Scope, Education and Experience, and Dimension criteria must be met to qualify for Advanced Practice Nurse (Nurse Practitioner/Clinical Nurse Specialist) II: Exception to Licensure for Graduate Nurse Technicians. Candidates who otherwise meet the basic requirements, but do not possess the required licensure and/or certification, may be appointed at the entry step of the Nurse II, as a Graduate Nurse Technician - Nurse Practitioner on a temporary appointment up to any period not-to-exceed one year and a day under the authority of 38 U.S.C. § 405(c)(2)(B). Nurse II is considered the entry grade level for APN (NP/CNS); no experience is required. Dimension Requirement: APN (NP/CNS) at level II are expected to meet the following dimension requirements: Clinical Practice. Delivers evidence-based health care, demonstrating proficiency in the ability to manage a patient panel, population, or program. Outcome Focus. The following Scope, Education and Experience, and Dimension criteria must be met to qualify for Advanced Practice Nurse (Nurse Practitioner/Clinical Nurse Specialist) III: Scope: Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care. Education and Experience Requirement: Meets basic education requirement for an APN (NP/CNS) AND one year of specialized APN (NP/CNS) experience equivalent to the Nurse II grade level. Dimension Requirement: APN (Nurse Practitioners/Clinical Nurse Specialist) at grade III are expected to meet the following dimension requirements: Clinical Practice. Influences and models the delivery of evidence- based health care, demonstrating expertise in the ability to manage a patient panel, population, or program. Outcome Focus. Leads and implements changes that enhance the quality and timeliness of Veteran/patient care applying research and evidence in the delivery of healthcare for patient populations. Leadership. Initiates and leads interdisciplinary teams informed by structured approaches and processes to improve outcomes that positively impact care for a patient panel, population, or program. Professional Development. Acquires knowledge and skills, then disseminates acquired information within and/or outside the medical facility. Professionalism/Collegiality. Preferred Experience: 1-2 years' experience as an APRN (NP/CNS) Wound and/or Ostomy certification is required Seeking and experienced full-time Wound Care APRN, with in-patient and outpatient work experience primarily focusing on the management of moderate to complex wounds and ostomy care Physical Requirements: Moderate lifting (15-44 lbs), use of fingers, both hands required, standing (2 hrs), repeated bending (2-4 hrs), see computer screen, depth perception, ability to distinguish basic colors, hearing (aid permitted). Environmental Factors: Working alone, Working closely with others. ["The Clinical Nurse Specialist(CNS)/Nurse Practitioner(NP), have prescriptive authority with a panel of patients. Core functions include diagnosing and treating acute and chronic health care problems, prescribing medication and treatments and is an independent practitioner providing quality and standard of care to a population of Veteran patients. Teaches health promotion and disease prevention to patients. Mentors and precepts students/trainees and those with lesser experience. Duties include but not limited to: Assesses, plans, implements and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. Applies the nursing process and care management principles within a collaborative, interdisciplinary practice setting, which will then enable effective implementation of health promotion and prevention practices, management of acute and chronic illnesses, and attainment of appropriate lengths of stay and the effective level of care. Core functions include daily assessment of patient care needs to determine the most appropriate use of expertise in clinical coordination, patient and staff education. Demonstrates consistent clinical competence in providing nursing care. Demonstrates evidence of critical thinking skills in identifying, analyzing, and resolving patient care problems on the unit. Initiates and implements an interdisciplinary plan of care based on assessments. Provides leadership in establishing goals, evaluating the plan of care, and in discharge planning. Initiates referrals to other disciplines and/or services as appropriate. Accurately documents in the medical record patient findings, assessments, and care provided. Documentation is technically informative, complete, accurate, consistent, concise, and timely. Effectively functions as the charge nurse, team leader, or in other leadership role that guides and directs others who provide patient care. Makes assignments and directs activities of other unit staff, (RNs, LPNs, students, PSAs, other team members). Uses a team approach to identify, analyze, and resolve patient care problems that result in improved patient outcomes.
