Position Highlights:
- Practice with a multi-disciplinary team of health care providers that includes physicians, NP’s, PA’s, RN case managers, social workers, and clinic staff.
- Our model is designed to provide a comprehensive, coordinated model of care to a predominantly Medicare-aged population.
- Primarily manage care in the patient’s home; however, care may require an office or other facility setting depending on the needs of the patient.
- Diagnose and treat both acute and chronic conditions such as diabetes, hypertension, kidney disease and other chronic medical conditions.
- Treat acute conditions such as minor injuries and other acute medical conditions such as infections or exacerbations of chronic diseases.
- Facilitate patient and family decision making regarding treatment decisions and end of life care and serious illness discussions.
- Prescribe pharmacological and non-pharmacological interventions.
- Practice with a multi-disciplinary team of health care providers that includes physicians, NP’s, PA’s, RN case managers, social workers, and clinic staff.

- Base salary plus annual quality bonus with a target of $50,000

Required Qualifications:
- Doctor of Medicine (M.D.), Doctor of Osteopathy (D.O.), or M.B.B.S
- Board certification or eligibility in internal medicine or family medicine
- Active, unrestricted medical license in Texas or the ability to obtain prior to employment
- Bilingual (English/Spanish) fluency
- Valid DEA license
- Valid Driver’s license and reliable transportation
- Doctor of Medicine (M.D.), Doctor of Osteopathy (D.O.), or M.B.B.S
Preferred Qualifications:
- Fellowship training in Palliative Care and/or Geriatrics
- A working understanding and support of a managed care environment
- Fellowship training in Palliative Care and/or Geriatrics
