Licensed Practical Nurse- Primary Care - PACT
The Patient Aligned Care Team (PACT) Licensed Practical Nurse (LPN) completes nursing intakes and provides nursing care to adult and geriatric patients within the Primary Care setting. Provides clinical support with PACT team to ensure an efficient workflow and develop comprehensive health care plans and management. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
