The Care in the Community Registered Nurse Coordinator, coordinates the episodic care of Veterans who need service(s) not able to be provided by the institution in a timely manner, in conjunction with the primary care provider and/or the physician managing the specialty service or Designated Approving Official (DAO). The RN CITC assesses and evaluates patient status and identifies priority needs for care; Serves as clinical resource for CITC Team members based on education and experience. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
