The Home Based Primary Care (HBPC) Nurse Practitioner position is located within within Home and Virtual Care Services, Nursing Service, VA Western New York Healthcare System. The HBPC NP is a professional nurse who with advanced clinical knowledge and competencies to function in a variety of roles. The HBPC NP is accountable for the management and evaluation of patient care for a panel of complex home bound veterans under their care. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
