This opportunity can be fully remote.IHANY is the Capital Region's first Clinically Integrated Network (CIN). They provide a collaboration among doctors, health systems and health care providers in New York's Capital Region aimed at improving the health of the communities they serve.This role is primarily responsible for leading the transformation of clinical care delivery for multiple CIN attributed populations with the overall goal of achieving the Triple Aim of improved patient experience of care, improved health of population, and reduce the per capita cost of healthcare.Fun
