POSITION TITLE: GERIATRICS CASE MANAGER GERIACTICS AND EXTENDED CARE Consultation and management of Veterans with multiple comorbidities, dementia, frailty and its associated complications who could benefit from an interdisciplinary team approach. Evaluates patients thru traditional face-to-face appointments, telephone visits, and virtual modalities approved by VHA.
