Registered Nurse Coordinator - Heart Failure Program (Wilmington)

The Heart Failure Program at ChristianaCare seeks a full-time Registered Nurse Coordinator to join their growing team as part of a planned expansion of Advanced Heart Failure and Heart Failure Disease Management Services to our Wilmington Hospital Campus.
The Registered Nurse will be part of an effort to establish and grow services to provide exceptional clinical care in the outpatient clinic including telemonitoring for high-risk patients, providing nursing level visits for IV Diuretic administration, providing counseling and education to patients and families/caregivers. The Registered Nurse will work as a key member of the Advanced Heart Failure Program to coordinate care for patients on the Wilmington Campus and Clinic. The Registered Nurse will work on program development efforts to establish disease management services.
The Clinical Duties will include
Provide in person and telephone based counseling and education to Advanced Heart Failure and LVAD patients and families.
Educates patients regarding health and illness prevention. Recommends community resources to meet patient and family needs.
Implements and monitors care plans. Monitors, records and communicates patient condition as appropriate.
Establish educational materials and methods to assess patients baseline knowledge and health and evaluate progress towards patient goals.
Coordinates care with home health and other providers as needed.
Triages patient and family phone calls and provides consultation.
Monitor patients receiving IV Diuretic Therapy in the clinic and update physicians regarding patient statuses in person or via telephone.
Oversee clinical work of the Medical Assistants ensuring accurate medicine reconciliation and medical histories are obtained for each visit.
Adjusts patient medications following CHF/LVAD medication protocols in collaboration with physician or and nurse practitioners.
Monitors patients with CardioMems implants per physician protocol.
Offers educational classes to patients and families on self-management skills, CHF symptoms, lifestyle choices.
Work Location & Hour :
Wilmington Hospital, Monday to Friday (8am to 4:30pm)
Education and Experience Requirement:
BSN, graduate of an accredited school of nursing
3 years clinical experience in Cardiac Care/Critical Care required.
An equivalent combination of education and experience may be substituted.
