As a Care Management Coordinator with us, you will have the opportunity to provide administrative support to our Care Management department.

Your day.
As a Care Management Coordinator you need to know how to:
· Serve as the administrative support person for the department by retrieving phone messages, collecting and distributing mail, ordering and maintaining office supplies, organizing meetings and recording minutes and maintaining resource lists. Schedule the department coverage. Take minutes at department meetings.
· Serve as the liaison between outside organizations including Payers, Physicians, home care agencies, Skilled Nursing Facilities, the Case Management department, families and other customers of the department. Document confirmation of post-acute care or other arrangements. Update and provide CM Coordinator with HC agency changes, SNF and/or transportation changes. Communicate to and educate patients regarding Home Care agency and visit expectations. Ensure agencies and facilities receive appropriate information to provide for smooth care transitions.
· Support case management operations by collecting and then faxing relevant patient information, benefits resources/contacts, and orders. Organize patient information to ensure patients receive timely home health care services. Maintain record of all post ⓠacute care arrangements made. Keep resource files up to date and enter new resources as acquired. Maintain appropriate communication with Revenue Service Center, Allscripts Software and other departments/ providers to facilitate the continuity of care.
· Maintain appropriate chart documentation. Maintain â˜Patient Choiceâ™ Letters. Ensure MD orders are complete and signed. Maintain the department database, web page and department lists. Update forms as needed. Assist with management of data and statistics within the department.
· Assist in care and maintenance of department equipment and supplies. Maintain clean unit, work space, and patientâ™s environment. Participate in quality initiatives, including patient satisfaction, patient care, and workflow
· Promote mission, vision, and values of SCL Health, and abide by service behavior standards.
· Perform other duties as assigned.
Your experience.

· High School Diploma or equivalent, required
· Minimum of two (2) years of experience in the healthcare field, required
· Associateâ™s Degree or higher education, preferred
· Prior experience in an acute care setting, preferred
